Bridging vertical and horizontal knowledge: a peer-education and mentorship model for adolescent addiction prevention

Adolescent substance use emerges within a transformed socio-cultural and epistemic landscape characterized by horizontal knowledge exchange, digital mediation, and intensified identity pressures. In this environment, prevention models grounded solely in vertical expert authority increasingly struggle to sustain credibility and engagement. Italian epidemiological data indicate earlier initiation of substance experimentation and rising rates of both substance-related and behavioral addictions. Developmental neuroscience highlights adolescents’ heightened sensitivity to peer evaluation, status dynamics, and reward processing, suggesting that effective prevention must be relational, autonomy-supportive, and identity-oriented. In response, we propose a hybrid prevention framework integrating structured scientific mentorship with peer education. Between 2022 and 2025, a multi-setting intervention was implemented in Italian secondary schools and sports organizations. Peer educators were selected for relational competencies and trained within a scientific mentor mindset emphasizing rigor, transparency, high expectations, and respect. The program combined interactive workshops, co-produced materials, and parallel involvement of teachers and parents within a bio-psycho-social developmental framework. This model aligns with core developmental mechanisms by redirecting status toward competence, strengthening prosocial belonging, enhancing emotional regulation, fostering agency, and reducing reactance through respectful adult positioning. In horizontally structured knowledge environments, prevention cannot rely exclusively on hierarchy nor on unstructured peer diffusion. A structured mentorship model embedded within peer networks offers an integrative pathway, positioning addiction prevention as a culturally responsive developmental strategy rather than solely a public health intervention.
